A new Hydroxychloroquine monitoring leaflet released

A new Hydroxychloroquine monitoring leaflet has been released for use by practices and other facilities that support this cohort of patients. Although practices may want to print out a copy for patients, a QR code is available which if scanned by a patients mobile phone, will take them to the complete document.

To access a printable version of the leaflet go to the 'Downloads' section of this website plus there is also a higher quality QR code that can be printed for patients to scan.

The leaflet has been uploaded onto the West Yorkshire Health and Care Partnership website (click here). along with shared care guidance for further reading. At this time there is no agreed pathway for specialised assessment or referral for optical practices.
